Job summary

Qualifications

• RN licensure in Kentucky and at least two years MDS experience in long-term care.
• LVN/LPN may be considered with exceptional qualifications.
• AANAC RAC-CT certification or ability to obtain within 90 days of hire.

Responsibilities

• Collaborate with facility team to assess resident needs.
• Coordinate care management meetings and set assessment reference dates.
• Ensure compliance with federal/state regulations for MDS.
• Monitor MDS and care plan documentation across residents.
• Maintain current MDS status per guidelines and track schedules.
• Enter resident data into computerized programs and ensure accurate coding.
• Attend team and other meetings to gather information and update records.
