Job summary

Kenwood Health Center, LLC in Richmond, KY is seeking an MDS Coordinator to ensure timely and accurate completion of the MDS process from admission to discharge, coordinating information systems and education for the clinical department.

The role requires LPN licensure in Kentucky with at least two years of MDS experience in long-term care, and either current AANAC RAC-CT certification or commitment to obtain it within 90 days of hire. Same day pay available.

Qualifications

• LPN licensure in Kentucky required.
• Minimum of two years MDS experience in long-term care.
• Current AANAC RAC-CT certification or obtain it within 90 days of hire.

Responsibilities

• Works in collaboration with facility team to assess the needs of the resident.
• Assists with coordination and management of care management meetings, including review of resident care and setting assessment reference dates.
• Complies with federal and state regulations regarding completion and coordination of the MDS process.
• Monitors MDS and care plan documentation for all residents.
• Ensures documentation is present in the medical record to support MDS coding.
• Maintains current MDS status of assigned residents according to state and federal guidelines.
• Supports the tracking system of MDS schedules (timeframes and due dates).
• Maintains frequent and accurate data entry of resident information into appropriate computerized programs.
• Completes accurate coding of the MDS with information from medical records, observations, and interviews.
• Attends team meetings and other meetings to gather information, communicate changes, and update records.
• Keeps knowledge base updated on data entry and computer technology.
• May participate in quality assurance activities.
• May complete electronic submission of required documentation to databases per company policy.
• Performs additional duties as assigned.

Education

LPN licensure in Kentucky
