Canterbury Nursing & Rehab is seeking an experienced MDS Coordinator to oversee the Resident Assessment Instrument (RAI) process and the interdisciplinary care planning, ensuring compliance with all state and federal regulations governing long‑term care.

You will lead clinical assessment efforts, coordinate teams, mentor staff, and promote personalized care plans that reflect resident needs and goals, in a supportive Indiana facility.

Qualifications

• Graduate of an accredited nursing program; BSN preferred.
• Minimum of 1 year in nursing management in long-term care.
• Two years of professional nursing experience in long-term care, acute care, restorative care or geriatric setting.

Responsibilities

• Coordinate and complete the RAI and interdisciplinary care planning process.
• Ensure compliance with state and federal regulatory requirements.
