Job summary

NHC HealthCare Bluffton is seeking a Registered Nurse (RN) to serve as MDS Coordinator. The role focuses on overseeing the Resident Assessment Instrument process, ensuring timely and accurate CMS/OBRA compliant assessments.

The MDS Coordinator will lead with compassion, coordinate with the interdisciplinary team, and monitor records to support MDS coding and patient care planning. This is a full-time on-site position at our Bluffton facility.

Qualifications

• Must have South Carolina RN license and be caring and compassionate.
• Excellent communication skills and a team player attitude.
• RN experience with MDS in a senior care setting preferred or LPN with MDS exposure.

Responsibilities

• Conduct and document accurate patient assessments.
• Interview patients and families to gather psychosocial and medical data.
• Coordinate with disciplines to ensure timely MDS completion.
• Schedule and meet deadlines, monitor medical records for MDS coding.
• Work with interdisciplinary team to finalize Resident Assessment Protocols.
• Provide in-services to center partners to ensure documentation accuracy.
• Communicate with bookkeeping as needed.
