Job summary

NHC HealthCare Bluffton is seeking an RN for the MDS Coordinator role to oversee the Resident Assessment Instrument process in compliance with CMS and OBRA guidelines. The RN will lead assessments, coordinate with interdisciplinary teams, and ensure timely completion of MDS and care plans.

Ideal candidates are licensed RN in South Carolina, with strong communication and organizational skills. The role offers full-time hours, competitive pay, and a benefits package including insurance and 401k.

Qualifications

• Must have South Carolina RN license.

Responsibilities

• Conduct and document accurate assessments of patients.
• Interview patients and families to obtain a full psychosocial/medical status.
• Coordinate with disciplines to ensure timely completion of the MDS.
• Schedule and meet deadlines for MDS activities.
• Monitor the medical record to validate MDS coding.
• Work with interdisciplinary team to complete Resident Assessment Protocols and Patient Care Plans.
• Support accuracy of medical record documentation through in-services to center partners.
• Communicate with the bookkeeping department.
