Job summary

Signature HealthCARE is seeking an MDS-focused Registered Nurse to coordinate resident assessments and MDS data collection. The role requires bedside involvement with families and collaboration across care teams to ensure compliant RAI processes.

The ideal candidate will have a current RN license, 3+ years in clinical healthcare, and at least 1 year of MDS experience, supporting continued high-quality patient outcomes in a facility setting.

Qualifications

• Registered Nurse with current state licensure.
• Minimum three (3) years of clinical experience in a health care setting; long-term care setting preferred.
• Minimum one (1) year of MDS experience.

Responsibilities

• Collect information to complete the MDS using the medical record, bedside assessment, and staff, resident and/or family interviews.
• Participate in Daily PPS meetings, weekly Medicare meetings, and month end meetings to assure federal billing requirements are met.
• Provide ongoing education of the RAI process to all interdisciplinary team members, direct and indirect caregivers, business office, etc. with all changes and as appropriate.
