Job summary

Marwood Nursing & Rehab is seeking an experienced RN MDS Coordinator to oversee Medicare MDS assessments and coordinate multidisciplinary care. This full-time position requires a Michigan RN license and MDS 3.0 certification.

The coordinator will collaborate with rehab, nursing and support staff to ensure excellence in quality of care and regulatory compliance, while supporting residents and families throughout their stay.

Qualifications

• Graduate from an accredited school of nursing.
• Must be licensed as a Registered Nurse in Michigan.
• Satisfactory performance in staff or charge nurse roles for at least 2 years.
• MDS 3.0 certification (or equivalent).
• Competent assessment skills and basic computer literacy.
• Strong organizational ability.
• Excellent oral and written communication skills.

Responsibilities

• Coordinate care for residents using a holistic approach.
• Collaborate with interdisciplinary team and families as needed.
• Be the primary team member with head-to-toe knowledge of residents' medical needs.
• Serve as a resource to floor staff for MDS education and support.
• Play an active role in discharge planning and communicate across disciplines.
• Assess residents' needs before discharge and address educational opportunities.
• Attend care plan meetings representing nursing.
• Meet daily with nurse manager, rehab director, social workers and dietitians.
• Performs other duties as assigned.
