Job summary

Rockport Healthcare Services seeks a Medicare/MDS Coordinator to lead the IDT in completing MDS assessments and ensuring compliance with state and federal regulations. The role requires an RN license and strong communication, with preference for CPR and long-term care experience.

In this position you will coordinate care plans, supervise relevant nursing interventions, and ensure timely documentation. You will uphold infection control standards and protect resident rights in a supportive center

Qualifications

• Current licensure in nursing (RN).
• Written and verbal communications skills in English as business necessity.
• Administrative and organizational ability and skills.
• Current CPR certification preferred.
• Two years nursing experience in long term care preferred.
• Supervisory experience preferred.

Responsibilities

• Coordinates the Medicare/MDS resident assessment process.
• Ensures the Interdisciplinary Team completes the MDS Assessment in a timely manner.
• Coordinates development, implementation and evaluation of plan of care.
• Coordinates and performs nursing interventions as indicated by the resident plan of care or as ordered by the physician.
• Provides nursing care in accordance with infection control standards.
• Follows safety policies in performing nursing care.
• Initiates emergency measures according to center policy and standards of nursing practice.
• Maintains essential information exchange for quality resident care.
• Ensures documentation complies with Federal/State regulations and Company policy.
• Participates in meetings and inservices.
• Represents professionalism to consumers and protects residents’ rights.
