Job summary

Villa at Green Lake Estates is seeking a MDS Coordinator RN to join their team. The MDS Coordinator RN will oversee the Resident Assessment Instrument process, coordinate MDS assessments, and develop care plans to support resident-centered care and compliant reimbursement.

This full-time role requires RN licensure in the state, CPR certification, and SNF MDS experience. You will work with an interdisciplinary team to ensure timely documentation and participate in QAPI initiatives.

Qualifications

• RN license to practice in the state where posting occurs.
• Current CPR certification is required.
• Experience as an MDS nurse in a skilled nursing facility is preferred.

Responsibilities

• Coordinate MDS assessments and ensure regulatory compliance.
• Facilitate resident care planning with the interdisciplinary team.
• Enter ICD-10 codes and support accurate reimbursement.
• Monitor changes in resident status and trigger necessary assessments.
• Participate in QAPI initiatives and staff education.
