Job summary

River Grove Health and Rehab invites qualified RNs or LPNs to join our MDS team in a coordinator role. You will oversee MDS/RAI processes, guide the interdisciplinary team, and ensure compliant documentation and timely submissions within a supportive LTC environment.

We value professional growth, CPR certification, and strong knowledge of MDS 3.0; experience in long-term care is preferred. This role offers a stable, full-time schedule at our Tennessee facility.

Qualifications

• Active RN/LPN license
• Certification in MDS
• Current CPR certification
• MDS 3.0 experience
• 3+ years clinical health-care experience, LTC preferred
• Current state licensure for LPN/RN

Responsibilities

• Ensure use of interdisciplinary rehab team process in MDS/RAI formulation.
• Observe direct nursing care, review documentation and make recommendations; assist with chart audits.
• Provide clinical support and direction to appropriate resource materials.
• Verify all MDS documents meet Federal/State guidelines or organizational policy and are completed timely.
• Participate in Daily PPS meetings, weekly Medicare meetings, and month-end meetings to ensure billing requirements are met.
• Assist in completing the Resident Assessment Instrument with the Interdisciplinary Team.
• Assist with coordination and completion of the Resident Assessment Instrument per Federal/State Regulations.
