Job summary

Village Health Care Center LLC is seeking an experienced MDS Coordinator to oversee total care administration and assist with the RAI/MDS process in compliance with state and federal requirements.

The role requires RN licensure, nursing degree, and EMR proficiency, with preferred background in long-term care and PCC experience. This in-person position offers benefits and growth opportunities within Evergreen Healthcare Group.

Qualifications

• RN licensure required.
• Graduate of an accredited school of nursing.
• Two years’ experience in long-term care preferred.
• Experience with RAI/MDS process and PCC preferred.

Responsibilities

• Coordinate the IDT to ensure timely completion of assessments.
• Plan and set MDS ARDs using MDS Scheduler.
• Communicate ARD selections for specific MDSs to IDT.
• Complete Care Area Assessments (CAAs) as required.
• Complete care plan items relevant to current MDS and attend care plan meetings.
• Monitor documentation in the medical record to validate MDS coding.
• Participate in Medicare utilization management as assigned.
• Stay up to date with RAI regulations.
