Job summary

Oak Grove Healthcare is seeking a qualified MDS Coordinator (RN) to oversee the Resident Assessment Instrument process and ensure accurate care planning and reimbursement. The role requires active RN licensure, attention to detail, and prior MDS experience within skilled nursing or rehabilitation settings.

You will work with an interdisciplinary team to optimize resident outcomes and maintain regulatory compliance, while embracing opportunities to improve processes and patient care.

Qualifications

• Active RN license in good standing.
• 6+ months of experience as an MDS Coordinator.
• Experience in Skilled Nursing/Rehabilitation preferred.
• RAC-CT or RNAC certification preferred.

Responsibilities

• Coordinate and participate in the completion of the MDS, CAA's, and Care Plan in compliance with Federal and State regulations.
• Monitor and document the management of Medicare and Managed Care residents with the interdisciplinary team.
• Ensure accuracy in resident assessments to maximize reimbursement and care quality.
• Drive innovation by implementing new ideas and processes to improve resident outcomes.
• Maintain compliance with evolving regulations and best practices.
