J. Morrissey & Company is seeking an MDS Coordinator to coordinate and manage MDS assessments in accordance with federal and state regulations.

This on-site or hybrid role ensures accurate clinical assessments, compliance with CMS guidelines, and development of individualized care plans to support quality resident outcomes. You will lead interdisciplinary care planning, monitor PDPM/reimbursement accuracy, and educate staff on MDS-related processes while staying current with regulatory changes

Qualifications

  • RN or LPN licensed in CT with active status.
  • MDS Coordinator Certification or willingness to obtain.
  • Minimum 1–2 years of clinical long-term care experience.
  • Strong knowledge of MDS 3.0, RAI, and CMS regulations.

Responsibilities

  • Coordinate, complete, and submit accurate MDS assessments (admission, quarterly, annual, significant change, discharge).
  • Ensure CMS/state/facility compliance with MDS and RAI processes.
  • Review clinical documentation to support MDS coding accuracy and reimbursement.
  • Monitor ARD schedules and assessment reference dates.
  • Lead or participate in interdisciplinary care plan meetings.

MDS COORDINATOR - DIRECT HIRE -On Site or Hybrid The MDS Coordinator is responsible for coordinating and managing the completion of the Minimum Data Set (MDS) assessments in accordance with federal and state regulations. This role ensures accurate clinical assessments, compliance with CMS guidelines, appropriate reimbursement, and the development of individualized care plans that support quality resident outcomes. Key Responsibilities MDS & Clinical AssessmentCoordinate, complete, and submit accurate and timely MDS assessments (admission, quarterly, annual, significant change, and discharge).Ensure compliance with CMS, state, and facility regulations related to MDS and RAI processes.Review clinical documentation to support MDS coding accuracy and reimbursement.Monitor and manage ARD schedules and assessment reference dates.Care Planning & Interdisciplinary CollaborationLead or participate in interdisciplinary care plan meetings.Collaborate with nursing, therapy, social services, dietary, and other disciplines to develop and update comprehensive care plans.Ensure care plans reflect current resident needs, preferences, and clinical status.Reimbursement & Quality MeasuresMonitor case mix, PDPM (or applicable payment model), and reimbursement accuracy.Identify clinical risks or missed opportunities affecting reimbursement.Monitor quality indicators and outcomes, and assist with quality improvement initiatives.Compliance & AuditsEnsure accurate documentation to support audits and surveys.Participate in state and federal surveys as related to MDS and care planning.Maintain current knowledge of regulatory changes affecting MDS and reimbursement.Education & SupportEducate staff on documentation requirements and MDS-related processes.Serve as a clinical resource for MDS, care planning, and reimbursement questions. Qualifications RequiredActive RN license in good standing in CTor Active LPN License in good standingMDS Coordinator Certification (e.g., RAC-CT, AANAC) or willingness to obtainMinimum of 1–2 years of clinical experience in long-term care or skilled nursingStrong knowledge of MDS 3.0, RAI process, and CMS regulationsPreferredPrior experience as an MDS CoordinatorExperience with PDPM reimbursement modelEMR/EHR experience (e.g., PointClickCare, MatrixCare) Skills & CompetenciesStrong attention to detail and organizational skillsAnalytical and problem-solving abilityExcellent written and verbal communicationAbility to work independently and as part of an interdisciplinary teamTime management and deadline-driven performance #INDH
