Job summary

CoreMedical Group is seeking an MDS Coordinator (RN or LPN) for a long-term care setting to oversee the RAI process and ensure timely, accurate MDS assessments. The ideal candidate has a strong understanding of LTC regulations, reimbursement, and interdisciplinary care planning.

The MDS Coordinator will work with nursing leadership and the interdisciplinary team to support quality outcomes, regulatory compliance, and accurate reimbursement.

Qualifications

• Active RN license preferred; LPN candidates will also be considered.
• Previous MDS Coordinator experience preferred
• Long-term care or skilled nursing experience required
• Experience with MatrixCare EMR strongly preferred
• Knowledge of Medicare, Medicaid, and MDS reimbursement processes

Responsibilities

• Complete and coordinate MDS assessments in accordance with federal and state regulations
• Develop and update resident care plans with the interdisciplinary team
• Ensure timely and accurate documentation to support reimbursement
• Monitor compliance with MDS scheduling and submission requirements
• Educate staff on documentation and regulatory standards
• Collaborate with clinical leadership to support quality improvement initiatives
