Job summary

The Blossoms is seeking a dedicated MDS Coordinator RN to conduct federally mandated assessments for residents at our long-term care facility. You will collect essential data to determine functional capacity, plan care, and support Medicare/Medicaid reimbursement across RUG-IV frameworks.

Qualifications

• RN license required.
• MDS experience preferred.
• Familiarity with OBRA, PPS and RUG-IV.

Responsibilities

• Complete OBRA, PPS, and Managed Care MDS.
• Prepare and maintain Admit MDS tracking forms and related documentation.
• Coordinate care plans and interdisciplinary team involvement.
• Schedule and conduct Care Plan conferences within set timelines.
• Maintain timely OBRA calendars and MDS schedules.
• Submit OBRA/PPS MDS to CMS per guidelines.
• Coordinate required certifications and Medicare parts documentation.
• Maintain ARD documentation and CAD/CAAs as required.
