Job summary

The Blossoms seeks an RN or LPN MDS Coordinator to lead federally mandated assessments for residents at a long-term care facility. You will collect data, compile comprehensive assessments, and determine care plans and reimbursement across payer sources per CMS RUG-IV guidelines.

Key duties include OBRA/PPS/MDS completion, coordinating care plans with the IDT, and maintaining EMR documentation to support ARD timelines and quality measures.

Qualifications

• RN or LPN with knowledge of MDS processes and CMS OBRA/PPS requirements.
• Ability to complete OBRA, PPS, and MDS assessments and related forms.
• Coordinate care plans and multidisciplinary team communications.
• Maintain documentation in EMR and ensure ARD timelines.
• Optional MDS Certification preferred.

Responsibilities

• Report to facility Administrator.
• Completion of all OBRA, PPS and Managed Care MDS.
• Coordinate Care Plan conferences and letters.
• Maintain OBRA calendars and schedules.
• Transmission of MDS assessments to CMS as required.
