SAK Management in Momence, IL is seeking a detail-oriented MDS Coordinator to oversee MDS assessments and care plans in compliance with federal and state regulations. The role involves coordinating resident assessments (MDS), ensuring accuracy, and collaborating with the interdisciplinary team to develop comprehensive care plans.

This full-time, in-person position requires an RN license and experience with PCC or similar electronic health records.

Competenze

• RN license is required and active.
• Experience coordinating MDS assessments and care plans.
• Knowledge of Medicare guidelines for MDS completion.

• Coordinate the development of resident MDS assessments in a timely manner.
