Job summary

Green Meadows Nursing Center is seeking an experienced MDS Coordinator to oversee timely completion of the RAI process and care management from admission to discharge. You will work under the direction of the Director of Nursing and VP of Clinical Reimbursement, coordinating information systems and education for the clinical team.

You will collaborate with an interdisciplinary team, ensure regulatory compliance, monitor MDS data, and support care planning processes.

Qualifications

• RN license in state of practice.
• Minimum 2 years nursing in a Skilled Nursing Facility.
• Strong knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement.
• Thorough understanding of OBRA regulations and Minimum Data Set (MDS).
• Experience with MDS 3.0 is preferred.

Responsibilities

• Collaborate with Interdisciplinary Team to assess resident needs and schedule MDS assessments and care plan reviews.
• Decide on necessity of additional MDS assessments based on clinical criteria and RAI Manual.
• Coordinate daily stand-up meetings reviewing resident care and assessment reference dates.
• Ensure compliance with federal/state regulations for MDS processes and coding.
• Maintain current MDS status and accurate data entry in computerized systems.
• Attend interdisciplinary meetings and communicate changes to staff and families.
• Assist DON with significant changes, physician orders, and verbal reports to reflect changes in MDS and care plans.
• Prepare scheduling and notices for care planning conferences; support DON with outcomes reporting.
