Job summary

Southview Acres Healthcare Center in St. Paul, MN is seeking an experienced MDS Coordinator (RN) to join our clinical leadership team. The role focuses on coordinating the RAI process to ensure accurate reimbursement and high-quality resident care.

You will lead MDS assessments, ensure timely submissions, and collaborate with interdisciplinary teams to optimize care plans. Strong knowledge of CMS Minnesota regulations and PDPM is essential.

Qualifications

• Strong knowledge of MDS and RAI processes.
• Familiarity with ICD-10 coding and PDPM impact on reimbursement.
• Experience in SNF or long-term care settings.
• Excellent communication and teamwork abilities.

Responsibilities

• Coordinate and complete MDS assessments (OBRA, PPS, state-specific).
• Ensure timeliness and accuracy of MDS documentation and submissions.
• Collaborate with care teams to develop and update care plans.
• Monitor case mix index (CMI) and opportunities for optimization.
• Ensure CMS and Minnesota state regulation compliance.
• Participate in care plan meetings and communicate with residents and families.
• Assist with audits, surveys, and quality initiatives.
• Stay updated on regulatory and reimbursement changes.
