You will lead MDS assessments, ensure timely submissions, and collaborate with interdisciplinary teams to optimize care plans. Strong knowledge of CMS Minnesota regulations and PDPM is essential.

Qualifications

• Strong knowledge of MDS and RAI processes.

Responsibilities

• Coordinate and complete MDS assessments (OBRA, PPS, state-specific).
• Ensure timeliness and accuracy of MDS documentation and submissions.
• Collaborate with care teams to develop and update care plans.
• Monitor case mix index (CMI) and opportunities for optimization.
• Ensure CMS and Minnesota state regulation compliance.
• Participate in care plan meetings and communicate with residents and families.
• Assist with audits, surveys, and quality initiatives.
• Stay updated on regulatory and reimbursement changes.

Job Title: MDS Coordinator (RN)

Location: Southview Acres Healthcare Center – St. Paul, MN

Employment Type: Full-Time

Position Summary

Southview Acres Healthcare Center is seeking a detail-oriented and experienced MDS Coordinator to join our clinical leadership team. The MDS Coordinator is responsible for coordinating and completing the Resident Assessment Instrument (RAI) process in accordance with federal and state regulations, ensuring accurate reimbursement, and supporting high-quality resident care.

Key Responsibilities

• Coordinate and complete MDS assessments (including OBRA, PPS, and any applicable state-specific assessments)
• Ensure accuracy and timeliness of all MDS documentation and submissions
• Collaborate with interdisciplinary team members to develop and update comprehensive care plans
• Monitor case mix index (CMI) and identify opportunities for optimization
• Ensure compliance with CMS guidelines and Minnesota state regulations
• Participate in care plan meetings and communicate effectively with residents and families
• Assist with audits, surveys, and quality improvement initiatives
• Stay up to date on regulatory changes and reimbursement updates

Qualifications

• Current RN license in the state of Minnesota
• Minimum 1–2 years of MDS experience preferred
• Strong knowledge of RAI process, ICD-10 coding, and PDPM
• Experience in a skilled nursing facility (SNF) or long-term care setting
• Strong organizational skills and attention to detail
• Excellent communication and teamwork abilities

Preferred Qualifications

• MDS Certification (RAC-CT)
• Experience with electronic health record (EHR) systems
