Job summary

Canton Center for Nursing and Healing, part of the Empire Care Centers family, is hiring an MDS Coordinator to ensure accurate resident assessments, CMS/RAI compliance, and timely QM/QI data analysis in a collaborative team.

The role requires an active Georgia RN license, prior MDS/RAI experience in long-term care, strong organizational and communication skills, and a commitment to high-quality care. A $5,000 sign-on bonus is offered along with comprehensive benefits.

Qualifications

• RN license in Georgia is required.
• Prior MDS/RAI experience in long-term care preferred.
• Strong organizational and communication skills are essential.
• Knowledge of CMS guidelines and care planning is necessary.
• Experience in long-term care is a plus.

Responsibilities

• Coordinate and oversee all MDS (Minimum Data Set) assessments per CMS/RAI guidelines.
• Collaborate with nurses, therapists, and department heads for accurate, timely documentation.
• Prepare and analyze QM/QI data for performance improvement.
• Ensure compliance with all RAI and MDS procedures and submission deadlines.

What You’ll Do

• Collaborate with nurses, therapists, and department heads to ensure accurate, timely documentation


• Prepare and analyze Quality Measure/Quality Indicator (QM/QI) data for performance improvement


• Ensure compliance with all RAI and MDS procedures and submission deadlines
