Job summary

Canton Center for Nursing and Healing in Georgia is seeking an experienced MDS Coordinator to ensure accurate resident assessments, compliant with CMS and LTCI guidelines, and to coordinate MDS processes in a collaborative, team-driven environment.

You will work closely with nurses, therapists, and department heads to maintain timely documentation, analyze QM/QI data, and support high-quality resident care while navigating regulatory requirements and deadlines.

Qualifications

• Current RN license in Georgia.
• Prior MDS/RAI experience in long-term care preferred.
• Strong organizational and communication skills.
• Knowledge of CMS guidelines and care planning.
• Long term care experience.

Responsibilities

• Coordinate and oversee all MDS (Minimum Data Set) assessments in accordance with CMS and LTCI guidelines.
• Collaborate with nurses, therapists, and department heads to ensure accurate, timely documentation.
• Prepare and analyze QM/QI data for performance improvement.
• Ensure compliance with all RAI and MDS procedures and submission deadlines.
