Job summary

Lake City Center for Nursing and Healing is seeking a dedicated MDS Coordinator. You will coordinate all MDS assessments, ensuring accurate documentation and CMS compliance within a team-driven environment.

The role emphasizes collaboration with nurses, therapists, and department heads, and focuses on data analysis for performance improvement in a long-term care setting.

Qualifications

• Current RN license in Georgia.
• Prior MDS/RAI experience in long-term care.
• Strong organizational and communication skills.
• Knowledge of CMS guidelines and care planning.
• Long term care experience.

Responsibilities

• Coordinate and oversee all MDS (Minimum Data Set) assessments in accordance with CMS and LTCI guidelines.
• Collaborate with nurses, therapists, and department heads to ensure accurate, timely documentation.
• Prepare and analyze Quality Measure/Quality Indicator (QM/QI) data for performance improvement.
• Ensure compliance with all RAI and MDS procedures and submission deadlines.
