Job summary

East Cobb Center for Nursing and Healing in Marietta, GA is seeking an experienced MDS Coordinator to join our clinical team. You will serve as the RAI/MDS Coordinator, ensuring accurate assessments and compliant documentation for residents while supporting high-quality care.

The ideal candidate is an RN or LPN with CMS/MDS knowledge, strong communication across disciplines, and a commitment to timely data transmission and quality measures.

Qualifications

• MDS eligibility and CMS/MDS guidelines knowledge required.
• Strong documentation and data accuracy skills required.
• Ability to work collaboratively with nursing and other disciplines.

Responsibilities

• Review and process MDS assessments for CMS/LTCI compliance.
• Coordinate with nursing and interdisciplinary teams to ensure accurate care plans.
• Ensure timely transmission of RAI/MDS data and generate QM/QI reports.
• Support quality improvement initiatives through data analysis.

MDS Coordinator Summary:

As the MDS Coordinator, you'll serve as the Resident Assessment Instrument (RAI)/Minimum Data Set (MDS) Coordinator for our center. Your responsibilities will include reviewing and processing MDS assessments in accordance with CMS and LTCI regulations and guidelines.Must be an RN or LPN

MDS Coordinator Responsibilities:

Your impactful contributions will include:

• Collaborating with nursing caregivers and other disciplines to ensure accurate care plan goals
• Effective communication with various disciplines to facilitate timely completion and transmission of RAI/MDS
• Preparing accurate reports on Quality Measure/Quality Indicators (QM/QI) from RAI/MDS data
