Job summary

Lake City Center for Nursing and Healing is seeking a dedicated MDS Coordinator to oversee all MDS assessments, ensuring accuracy, timely documentation, and compliance with CMS and LTCI guidelines.

You will collaborate with nursing, therapy, and department heads to drive quality care and data-driven improvements while staying aligned with state requirements and agency policies.

Qualifications

• RN license in Georgia is required.
• Experience with MDS/RAI in long-term care preferred.
• Knowledge of CMS guidelines and care planning.
• Strong organizational and communication abilities.

Responsibilities

• Coordinate and oversee all MDS assessments per CMS and LTCI guidelines.
• Collaborate with nurses, therapists, and department heads for accurate, timely documentation.
• Prepare and analyze QM/QI data for performance improvement.
• Ensure compliance with RAI and MDS procedures and submission deadlines.

What You’ll Do

• Coordinate and oversee all MDS (Minimum Data Set) assessments in accordance with CMS and LTCI guidelines
• Collaborate with nurses, therapists, and department heads to ensure accurate, timely documentation
• Prepare and analyze Quality Measure/Quality Indicator (QM/QI) data for performance improvement
• Ensure compliance with all RAI and MDS procedures and submission deadlines

Qualifications
