Job summary

Arcadia-Ltc- in Bloomington, IL is seeking an MDS Coordinator to ensure accurate and timely completion of Medicare/Medicaid MDS and care plan documentation for optimal reimbursement.

You will oversee MDS/PPS, care plans, restorative nursing coordination, and interdisciplinary collaboration with nursing, dietary, social services, and physicians. Requirements include RN or LPN licensure, LTC experience, restorative certification within 6 months, and strong English communication.

Qualifications

• Registered Nurse (RN) or Licensed Practical Nurse (LPN) with current licensure.
• Long Term Care experience required.
• Restorative certification due within 6 months if not already certified.
• Ability to read/write/speak/understand English.

Responsibilities

• Assess and determine health status and care level for new admissions.
• Ensure timely completion of MDS assessments (PPS Medicare, quarterly, annual).
• Maintain an accurate MDS reference schedule throughout resident stay.
• Coordinate care conferences and MDS reviews per regulations.
• Oversee restorative nursing measures on care plans.
