Job summary

Arcadia Care in Springfield, IL is seeking an experienced MDS Coordinator to ensure accurate Medicare/Medicaid case-mix documentation and optimal PDPM/RUG categorization. You will oversee the MDS PPS process, care plans, and the restorative nursing program, collaborating with nursing, dietary, social recreation and physicians to maximize reimbursement.

The ideal candidate is a Registered Nurse or Licensed Practical Nurse with long-term care experience and restorative certification, able to read,

Qualifications

• Must be a Registered Nurse or Licensed Practical Nurse with current state licensure.
• Long term care experience required.
• Restorative certification required within six months if not already certified.
• Ability to read, write, speak and understand English.

Responsibilities

• MDS Assessment and Documentation: assess health status and complete PPS Medicare MDS on time.
• Care Planning and Resident Assessment: complete care plans per regulatory time frames.
• Care Conferences and Interdisciplinary Coordination: schedule and conduct conferences and reviews.
• Restorative Nursing Program Coordination: reflect restorative measures on care plans.
