Arcadia Care is seeking an experienced MDS Coordinator for an in-person role in Illinois. You will oversee MDS assessments, care plans, and PPS/PDPM documentation to ensure proper reimbursement and regulatory compliance.

You will collaborate with nursing, dietary, social services, restorative and therapy teams to coordinate resident assessments, develop care plans, and lead interdisciplinary review meetings. RN or LPN licensure is required.

Qualifications

• Current, unencumbered RN or LPN license.
• Long Term Care experience required.
• Restorative certification within 6 months of hire if not already certified.
• Ability to read, write, speak, and understand English.

Responsabilités

• Perform MDS assessment and documentation to ensure accurate PPS/PDPM reimbursement.
• Develop and update care plans on admission and per regulatory timelines.
• Coordinate resident care conferences with interdisciplinary team.
• Oversee restorative nursing program documentation and integration into care plans.
