Arcadia LTC in Lincoln, IL is seeking an MDS Coordinator for an in-person role responsible for Medicare/Medicaid case-mix and care plan documentation to assure proper reimbursement. You will oversee MDS/PPS documentation, care plans, and restorative nursing alignment across disciplines.

Qualifications include RN or LPN licensure, long-term care experience, restorative certification within six months, and strong English communication. No on-call benefits and room for leadership development.

Formación

• RN or LPN licensure (state-registered) is required.
• Long Term Care experience is required.
• Restorative Certification must be obtained within 6 months of hire if not already certified.
• Ability to read, write, speak and understand English.

Responsabilidades

• MDS assessment and documentation for PPS/Medicare and Medicaid.
• Care planning and resident assessment per regulatory timeframes.
• Coordinate care conferences and interdisciplinary reviews.
• Oversee restorative nursing documentation on care plans.

Educación

Registered Nurse (RN) or Licensed Practical Nurse (LPN) license
