Job summary

Arcadia Care in Illinois is seeking an MDS Coordinator to oversee accurate Medicare/Medicaid case-mix and care plan documentation to support appropriate reimbursement. You will continually review MDS and care plans to optimize PDPM and RUG categorization and ensure timely submissions.

You will lead MDS assessments, care planning, and interdisciplinary conferences, coordinating restorative nursing and collaborating with nursing, dietary, social, recreation, rehabilitation and physician services

Qualifications

• Must be an RN or LPN with current, unencumbered state licensure.
• Long-term care experience required.
• Restorative certification required within six months of hire if not already certified.
• Ability to read, write, speak and understand English.

Responsibilities

• Assess health status and care level for all new admissions.
• Ensure timely completion of MDS assessments including PPS Medicare, quarterly, annual, and significant changes.
• Complete care plans on admission, quarterly and as needed within regulatory timeframes.
• Schedule and conduct resident care conferences and ensure MDS reviews are completed before conferences.
• Oversee restorative nursing measures reflected on the care plan and coordinate with the interdisciplinary team.
