Job summary

SupportFinity™ is seeking an experienced MDS Coordinator based in New Mexico. This position requires collaboration with the Interdisciplinary Team and compliance with federal and state regulations regarding the Minimum Data Set and care management.

The ideal candidate should have a nursing background, preferably with experience in a Skilled Nursing Facility, and knowledge of Medicaid reimbursement and Quality Indicator processes. The role offers full-time hours and benefits, including healthcare and PTO.

Qualifications

• Graduate of an approved Registered Nurse program and licensed in the state of practice.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.
• Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement.

Responsibilities

• Coordinate MDS assessments and care plan reviews as required.
• Ensure compliance with federal and state regulations regarding the RAI process.
• Maintain current MDS status of assigned residents according to guidelines.
