Job summary

Wellington Care Center is seeking an MDS Coordinator to ensure accurate assessments, compliance with Federal and State regulations, and effective care planning for residents. You will coordinate multiple MDS types, lead care-team meetings, and monitor documentation to support quality outcomes.

The role requires strong organizational skills, reliability, and the ability to work well with interdisciplinary teams.

Qualifications

• For this role, a current Nursing License in good standing is required.
• Experience in geriatric nursing and resident-centered care is preferred.
• Strong assessment, documentation, and communication skills are essential.
• Knowledge of nursing home regulations and compliance is needed.
• Ability to work independently and within a team, with multitasking ability.

Responsibilities

• Coordinate admission, annual, quarterly, Medicare, and change‑of‑condition MDS assessments.
• Ensure timely communication with nursing and other departments.
• Monitor MDS and care plan documentation for accuracy and completeness.
• Maintain expertise in MDS software for data entry and reporting.
• Lead interdisciplinary meetings to coordinate Medicaid, ITTM, and care plans.
• Update care plans based on current assessments and resident status.
• Generate and distribute Quality Indicators Reports to leadership.
• Submit MDS data per reporting standards in a timely manner.
• Stay current with industry standards through professional development.
• Oversee ancillary data and report to corporate billing for accuracy.
