Job summary

Creative Solutions in Healthcare, Inc. is seeking an MDS Coordinator to ensure accurate assessments and compliant care planning for residents. This role requires reliability, meticulous organization, and collaboration across nursing, IT, and billing teams.

You will oversee admissions, annuals, change-of-condition assessments, and data reporting while maintaining up‑to‑date knowledge of federal and state guidelines and MDS software proficiency.

Qualifications

• Current Nursing License in good standing.
• Experience in geriatric nursing.
• Excellent assessment, documentation, and communication skills.
• Understanding of nursing home regulations and compliance requirements.
• Ability to work independently and in a team.
• Ability to manage multiple priorities with accuracy and efficiency.

Responsibilities

• Coordinate resident assessments (admission, annual, quarterly, Medicare, change-of-condition).
• Ensure regulatory compliance with Federal and State guidelines for MDS.
• Monitor documentation in MDS and care plans for accuracy and organization.
• Maintain proficiency in MDS software for data entry and reporting.
• Lead interdisciplinary meetings (Medicaid, ITTM, care planning).
• Update and revise care plans based on current assessments and resident status.
• Generate and distribute Quality Indicators Reports to leadership.
• Submit MDS data per reporting standards.
• Attend professional development sessions to stay current with industry standards.
• Oversee ancillary data and report to corporate billing.
