Job summary

Creative Solutions in Healthcare - South is seeking an experienced MDS Coordinator to ensure accurate assessments, regulatory compliance, and coordinated care planning for residents in our facility.

You will lead interdisciplinary meetings, manage MDS data entry, update care plans, and generate quality indicators for leadership, while staying current with state and federal requirements and advancing resident-centered care.

Qualifications

• Hold a current Nursing License in good standing.
• Experience in geriatric nursing and resident-centered care.
• Strong assessment, documentation, and communication skills.
• Knowledge of nursing home regulations and compliance requirements.
• Ability to manage multiple priorities independently and with a team.

Responsibilities

• Coordinate admission, annual, quarterly, Medicare, and change-of-condition assessments with nursing and other departments.
• Ensure compliance with Federal and State guidelines for MDS.
• Review MDS and care plans for accuracy and organization.
• Maintain expertise in MDS software for data entry and reporting.
• Attend and coordinate Medicaid, ITTM, and care plan meetings.
• Update care plans based on current assessments and resident status.
• Generate and distribute Quality Indicators Reports to leadership.
• Submit MDS data per reporting standards.
• Pursue ongoing professional development to stay current with industry standards.
• Oversee ancillary data reporting to corporate billing.
