Job summary

Arcadia Care is seeking an MDS Coordinator to ensure accurate and timely Medicare/Medicaid MDS and care plan documentation, optimizing PDPM and RUG categorization. You will oversee MDS/PPS submission and coordinate the restorative nursing program within a multi-disciplinary team.

Responsibilities include MDS assessments, care planning, scheduling reference dates, and facilitating interdisciplinary care conferences to ensure compliance and proper reimbursement.

Qualifications

• Must be a Registered Nurse or Licensed Practical Nurse with current unencumbered state licensure.
• Long Term Care experience required.
• Restorative Certification required within six (6) months of hire if not already certified at time of employment.
• Ability to read, write, speak and understand the English language.

Responsibilities

• MDS assessment and documentation for resident care.
• Assess health status and level of care for new admissions.
• Maintain schedule of all MDS assessments with reference dates.
• Care planning and resident assessment per regulatory time frames.
• Schedule and conduct resident care conferences in compliance with regulations.
• Oversee that restorative nursing measures are reflected on the care plan.
