Arc at Normal is seeking an MDS Coordinator to ensure accurate Medicare/Medicaid documentation and optimal reimbursement. You will oversee MDS, PPS, and care plans, coordinating with nursing, dietary, social, and physician services to support resident care and reimbursement outcomes.

The role requires RN or LPN licensure, LTC experience, and restorative certification within six months. In-person, on-site work with opportunities for leadership development and employee support programs.

Formación

• RN or LPN with current state licensure and LTC experience.
• Experience with MDS, PPS, PDPM, and care planning processes.
• Ability to read, write, speak and understand English clearly.

Responsabilidades

• Assess health status and level of care for new admissions.
• Ensure timely completion of MDS assessments and PPS documentation.
• Coordinate care plans on admission and at regulatory timeframes.
• Schedule and conduct interdisciplinary care conferences.
• Oversee restorative nursing documentation and integration into care plans.
