Job summary

Arcadia Care in Springfield, IL seeks an experienced MDS Coordinator to ensure accurate Medicare/Medicaid case-mix documentation and timely PDPM/RUG classification. You will oversee MDS/PPS documentation, care plans, and restorative nursing coordination across disciplines.

The role requires active RN or LPN licensure, long-term care experience, and timely, compliant documentation. Strong interdisciplinary collaboration and attention to regulatory standards are essential.

Qualifications

• Must be a RN or LPN with current state licensure.
• Long-term care experience required.
• Restorative Certification within 6 months if not already certified.
• Ability to read, write, speak and understand English.

Responsibilities

• Assesses health status and level of care for all new admissions.
• Ensures accurate and timely completion of MDS Assessments (PPS/Medicare).
• Maintains an accurate schedule of all MDS assessments with correct reference dates.
• Completes care plans on admission, quarterly and as needed.
• Schedules and conducts resident care conferences with regulatory compliance.
• Oversees restorative nursing measures reflected on the care plan.

Education

Registered Nurse or Licensed Practical Nurse with current unencumbered state licensure.
