Job summary

Arcadia on the Hill is seeking an MDS Coordinator to ensure accurate and timely Medicare/Medicaid case-mix documentation and care plans, supporting optimal PDPM and RUG categorization. You will lead MDS assessments, care plan development, and interdisciplinary coordination, integrating nursing, dietary, social services, rehabilitation, and physicians to secure proper reimbursement.

Preferred candidates are RNs or LPNs with long-term care experience and restorative certification within six

Qualifications

• RN or LPN with current unencumbered state licensure.
• Long Term Care experience required.
• Restorative Certification within six months of hire if not already certified at time of employment.
• Ability to read, write, speak and understand English.

Responsibilities

• MDS Assessment and Documentation: assess health status of new admissions and ensure timely completion of all MDS assessments (PPS Medicare, quarterly, annual, significant change).
• Care Planning and Resident Assessment: complete care plans on admission, quarterly and as needed; ensure documentation is complete and thorough.
• Care Conferences and Interdisciplinary Coordination: schedule and conduct resident care conferences and ensure MDS reviews are completed before conferences.
• Restorative Nursing Program Coordination: oversee restorative nursing measures reflected on the care plan.

Education

Registered Nurse or Licensed Practical Nurse licensure
