Job summary

The Orchards is seeking an MDS Coordinator to oversee RAI, MDS assessments, PPS, CAAs, and care planning for residents. This role ensures regulatory compliance with Medicare/Medicaid and OBRA, while maximizing reimbursement opportunities and supporting quality outcomes.

The MDS Coordinator collaborates with the interdisciplinary team, leads the care planning process, and educates staff on documentation standards.

Qualifications

• Graduate of an accredited School of Nursing.
• Current LPN or RN license in the state where the building is located.
• Minimum of one year of nursing experience; MDS coordination experience preferred.
• Knowledge of Medicare, Medicaid, OBRA, PPS, Case Mix, and Quality Measures preferred.
• Strong assessment, organizational, analytical, and time management skills.

Responsibilities

• Coordinate MDS assessments, PPS documentation, CAAs, and care plans in accordance with federal, state, and facility regulations.
• Lead the interdisciplinary team in developing, revising, and implementing individualized resident care plans.
• Monitor Medicare certifications, skilled nursing utilization, and resident reimbursement to ensure appropriate documentation and reimbursement.
• Audit MDS assessments, CAAs, and care plans for accuracy and regulatory compliance.
• Provide education and training to nursing staff regarding MDS requirements and regulatory compliance.
• Maintain electronic documentation and assist with admission, discharge, re-entry, and Medicare tracking responsibilities.
