Job summary

Blossom Care Center seeks an LVN or RN to join as MDS Coordinator, guiding care through accurate MDS/CAAs and ARD planning. You will collaborate with an interdisciplinary team, schedule OBRA/Medicare/Medicaid assessments, and maintain documentation to reflect residents' needs.

Experience with PointClickCare is preferred. The role requires strong organizational and leadership abilities to manage multiple tasks while ensuring compliance and resident wellbeing.

Qualifications

• Must possess a current, unencumbered, active license to practice as an LVN or RN in the state.
• 2 years' experience in Long-Term Care preferred.
• Experience with RAI process completion preferred.

Responsibilities

• Maintain the current MDS status of each resident in accordance with laws, rules, and facility policies.
• Schedule residents for OBRA, Medicare PPS, and Medicaid assessments; communicate schedules to departments and families.
• Monitor clinical systems and coordinate MDS reassessment and care plan updates as resident conditions change.
• Set ARD with the interdisciplinary team to capture needs and services.
• Complete MDS and CAAs accurately per guidelines; participate in required meetings.
