Job summary

The Blossoms is seeking a Registered Nurse to serve as MDS Coordinator for our long-term care facility. The RN conducts federally mandated assessments and compiles data to determine functional capacity and care plans to support appropriate reimbursement under CMS guidelines.

You will manage OBRA, PPS, and MDS processes, coordinate care plans with the multidisciplinary team, and prepare CMS/payer documentation with strong organizational skills.

Qualifications

• RN licensure required to practice in the state.
• Experience with MDS, OBRA, PPS and CAAs preferred.

Responsibilities

• Coordinate OBRA, PPS, and MDS assessments.
• Develop and monitor care plans and coordinate with IDT to ensure timely updates.
• Prepare and transmit OBRA/MDS documentation to CMS and payers.
• Ensure ARD periods, CAAs and requisite MDS interviews are completed.

MDS Certification (AANAC) - optional
