Job summary

The Blossoms in Little Rock, AR is looking for an experienced MDS Coordinator/Nurse (RN or LPN) to perform federally required assessments for residents in a long-term care facility. This role requires accuracy and adherence to OBRA, PPS and MDS timelines.

You will manage Care Plans with the interdisciplinary team, coordinate OBRA schedules, ensure ARD/date compliance, and support CMS reporting and RUG-IV documentation with attention to detail.

Qualifications

• Registered Nurse (RN) or Licensed Practical Nurse (LPN) qualification is required.
• Familiarity with OBRA, PPS, MDS processes and CMS reporting.

Responsibilities

• Conduct federally mandated assessments for residents.
• Coordinate OBRA, PPS and MDS workflows and documentation.
• Develop and update Care Plans with the IDT.
• Ensure timely completion of MDS interviews and ARD deadlines.
• Prepare reports for CMS, RUG-IV, and billing.
• Maintain accurate ADL coding and EMR entries.
