Front Porch in Santa Rosa, CA is seeking a MDS/RAC Coordinator to coordinate resident assessments (MDS, RAPs, RAP summary) and oversee timely completion of comprehensive assessments at admission, annually and with significant condition changes.

Under the Director of Nursing, you will ensure interdisciplinary plans of care, schedule conferences, monitor progress, provide training to staff, and maintain documentation in compliance with OBRA/Regulations.

Qualificações

• Licensed Vocational Nurse in California.
• CPR and First Aid certifications required.
• 1–2 years PDPM MDS experience preferred.

Responsabilidades

• MDS Coordinator coordinating resident assessments and data.
• Ensure interdisciplinary assessments and care plans meet OBRA timelines.
• Schedule interdisciplinary conferences for care plan reviews.
• Monitor care plans and ensure use in interdisciplinary care.
• Provide training and clinical supervision in resident assessment.
• Maintain documentation per state/federal regulations and policy.
