Job summary

Front Porch in Santa Rosa, CA is seeking a Nurse/Nurse Coordinator to lead RAC/MDS processes. The role requires coordination of assessments, RAPs, and care plans, with attention to regulatory compliance and quality outcomes.

The MDS Coordinator will train staff, manage data transmission, and ensure accurate documentation. This full-time hourly position includes benefits and a Tuesday–Saturday schedule, with a pay range of $55.25–$60.00 per hour.

Qualifications

• Must have good communication and customer service skills; ability to work well with others and take direction.
• Ability to work under pressure to ensure timely review, correction and reporting of payroll.
• This position will also assist accounts payable and accounts receivable with imputing data and deposits.

Responsibilities

• Coordinate the resident comprehensive assessment (RAC) including MDS, RAPs and RAP summary sheet.
• Schedule interdisciplinary conferences for review and updating of resident assessment and care plan.
• Monitor and adjust plans of care reflecting current resident needs within OBRA regulations.
• Provide educational training and clinical supervision in resident assessment for staff.
• Ensure documentation for the resident assessment process is compliant with state and federal rules.

Summary

Under the direction of the DON, the MDS/RAC is responsible for conducting and coordinating the completion of the resident comprehensive assessment (RAC) which includes the Minimum Data Set (MDS), Resident Assessment Protocols (RAPs), and the RAP summary sheet. The coordination of periodical comprehensive, interdisciplinary assessments upon admissions, annually and whenever there is significant change of condition.

Detailed responsibilities

• MDS Coordinator
• Provides quality Customer Service efficiently to residents, families, co-workers and vendors in a manner to ensure satisfaction
• Ensures interdisciplinary, comprehensive assessments and care plans are completed using common assessment periods within the times frames established by OBRA regulation
• Schedules interdisciplinary conference for review and updating of resident assessment and care plan
• Monitors plans of care to ensure resident assessments are utilized in the development of an interdisciplinary plan of care reflective of the resident’s current physical, mental and psychosocial needs
• Provides educational training and clinical supervision in resident assessment for appropriate health care workers
• Assists Resident Assessment Coordinators (MDS) with computerized medical data and transmission
• Ensures documentation relative to resident assessment process (RAI) is maintained according to State and Federal regulations and company policy and procedure
• Communicates problem identification with resident assessment process promptly to the DON and Executive Director
• Conducts and participates in PDPM meetings
• Reports data collection, trends, QSAP reports and attends QA meetings
• Performs all duties in a safe and efficient manner Wears safety equipment as required for the job Reports any safety hazards and/or accidents to supervisor
• Attends all mandatory in-service meetings Complies with all department and facilities policies and procedures

Competencies and skills

Essential:
