Santa Rosa Post Acute in Santa Rosa, CA seeks an RN or LVN to oversee the facility's resident assessment process, including MDS coordination and PDPM compliance. Start ASAP; full-time position.

Responsibilities include supervising weekly PDPM meetings, tracking QM/QASP performance, providing PDPM and MDS training, ensuring timely MDS submissions, and coordinating interdisciplinary care to maintain regulatory standards.

Formación

• Requires RN or LVN with BSN preferred.
• Must be in good standing with the State Board of Nursing and maintain continuing education/licensing.
• Minimum 3 years of supervisory experience; long-term care experience preferred.

Responsabilidades

• Oversee the facility's resident assessment process, including MDS assessments and care planning.
• Lead weekly PDPM meetings for skilled patients and monitor tool utilization.
• Provide PDPM and MDS training to new coordinators and staff.
• Coordinate interdisciplinary participation for MDS within regulatory timeframes.
• Ensure confidentiality and accurate data entry/submission of MDS records.
