Job summary

Fundamental is seeking an MDS Nurse (LVN) in Mansfield, TX to coordinate the Resident Assessment Instrument (RAI) process, prepare accurate MDS data, and help develop the Interdisciplinary Plan of Care under the DON.

The role requires an active LVN license, 1+ year experience with MDS/RAP, and the ability to work with the healthcare team; we offer competitive hourly wages, comprehensive benefits, and a supportive work environment.

Qualifications

• Must have a current, unencumbered LVN license.
• 1+ years of direct hands-on nursing and MDS/RAP experience preferred.
• Must be able to utilize local and consultant resources in the execution of job responsibilities.
• Must have knowledge of state, federal, and regulatory requirements related to the job.
• Reliable transportation required.

Responsibilities

• Coordinating the Resident Assessment Instrument (RAI) process
• Completing an accurate Minimum Data Set (MDS) and development of the Interdisciplinary Plan of Care
• Participating in the daily Medicare Prospective Payment System (PPS) Interdisciplinary Team Meeting (ITM)
• Establishing and maintaining positive relationships with all members of the healthcare team and functioning as part of the nursing management team.
