Job summary

The Center at Cordera is seeking a full-time LVN MDS Coordinator to develop and coordinate MDS, CAAs, and Care Plans in accordance with state and federal requirements. You will join the facility’s Interdisciplinary Team to ensure timely assessments and accurate documentation.

Ideal candidates have LVN licensure, at least two years in skilled or long-term care, and experience with the RAI/MDS process; MDS certification is preferred.

Qualifications

• LVN license in good standing.
• 2+ years in skilled or long-term care.
• Experience with RAI/MDS process; MDS certification preferred.
• Ability to coordinate IDT and complete MDS/CAAs and care plans.

Responsibilities

• Coordinate IDT to timely complete MDS and CAAs.
• Plan ARDs using MDS Scheduler.
• Communicate ARD selections to IDT.
• Complete MDS sections and CAAs as assigned.
• Complete Care Plan items and attend meetings.
• Monitor documentation to support MDS coding.
• Participate in skilled utilization and state case mix management.
• Teach MDS item completion to IDT and CNAs.

We are looking for a FT LVN MDS to join our Team.Job descriptionPURPOSE OF JOB POSITION:The primary purpose of this position is to conduct and coordinate the development and completion of the Resident Assessment Instrument (RAI), that is, the Minimum Data Set (MDS), Care Are Assessments (CAAs) and Care Plan in accordance with State and Federal requirements. The MDS Coordinator participates as part of the facility Interdisciplinary Team (IDT) in the systems and processes to manage patients receiving skilled services as assigned. Performs other nursing duties as assigned.ESSENTIAL FUNCTIONS:· Coordinates the interdisciplinary team in timely completion of the assessments· Utilizes MDS Scheduler to plan and set MDS Assessment Reference Dates (ARDs)· Communicates ARD selection for specific MDSs to IDT· Completes the MDS sections as assigned· Completes CAAs as required· Completes Care Plan items and attends Care Plan meetings as assigned· Monitors overall the documentation in the medical record to validate that it supports MDS coding· Participates in skilled utilization and State Case Mix management as assigned· Provides teaching and training for MDS item completion to interdisciplinary team members that have responsibility for MDS item completion· Provides teaching and mentoring of C.N.A. staff along with Director of Nurses and Staff Development related to completion of the Nursing ADL documentationOTHER DUTIES:· Performs nursing duties as assignedEDUCATION REQUIREMENTSLVN· At least two years’ experience working in a skilled or long term care setting· Experience with RAI process and MDS certification preferred''Work RemotelyNo
