Job summary

Priority Management is seeking a licensed nurse (LPN/LVN or RN) to join our care team at Mansfield Medical Lodge. You will handle MDS responsibilities, care management from admission to discharge, and ensure compliance with federal, state and facility guidelines.

The role emphasizes rehabilitation and restorative nursing, coordination with the clinical reimbursement group, and ongoing education for the care team.

Qualifications

• Three (3) years' MDS experience in a nursing care facility preferred.
• Must possess a current, unencumbered, active license to practice as an LPN/LVN or RN in this state.
• Must have training in rehabilitative and restorative nursing practices.
• Must have a working knowledge of managed care rules, regulations, and guidelines.

Responsibilities

• Responsible for timely and accurate completion of both the RAI process and care management process from admission to discharge in accordance with company policy and procedures, and Federal, State and Certification guidelines.
• With direction from the VP of Clinical Reimbursement, may coordinate information systems operations and education for the clinical department.
