Job summary

The Pavilion at Creekwood is seeking an MDS Nurse (LVN) to coordinate the RAI process and develop an accurate MDS as part of the Interdisciplinary Plan of Care. This role supports the DON and nursing management team in a on-site facility setting in Texas.

Qualified candidates will hold a current LVN license, have 1+ year of direct nursing and MDS/RAP experience, and demonstrate strong collaboration with the healthcare team. Organization-wide benefits are provided for full-time staff.

Qualifications

• Must have a current, unencumbered LVN license as required by state.
• 1+ years of direct hands-on nursing and MDS/RAP experience preferred.
• Reliable transportation required.

Responsibilities

• Coordinate the Resident Assessment Instrument (RAI) process.
• Complete an accurate Minimum Data Set (MDS) and contribute to the interdisciplinary Plan of Care.
• Participate in the daily Medicare Prospective Payment System (PPS) Interdisciplinary Team Meeting (ITM).
• Establish and maintain positive relationships with all members of the healthcare team as part of the nursing management team.
