Job summary

Brickyard Healthcare seeks an experienced MDS Coordinator (LPN or RN) to coordinate the resident assessment process in line with federal and state requirements and company policies.

The role requires an active LPN or RN license, CPR certification, and at least 2 years of clinical experience, including 1 year in long-term care. You will collaborate with physicians, families, payers, and staff to ensure compliant MDS delivery.

Qualifications

• Licensed Practical Nurse with a current, active license in the state of practice.
• Must hold and maintain a current CPR certification.
• Minimum two (2) years of clinical experience in a health care setting.
• Minimum of one (1) year of experience in a long term care setting.
• Prior experience as an MDS coordinator with demonstrated expertise.
• Must be capable of maintaining regular attendance.

Responsibilities

• Completion of the Resident Assessment Instrument (RAI/MDS) per federal and state regulations and company policy.
• Act as in-house case manager by coordinating services with physicians, families, third party payers and facility staff.
• Coordinate the development and completion of the resident assessment (MDS) per current rules and guidelines, including RAPs and Triggers.
• Select the ARD and appropriate reasons for assessment to capture resources used for Medicare/Medicaid billing.
• Use company systems to manage the MDS process.
• Assist in developing a comprehensive resident assessment and care plan.
• Communicate with product champions to train new nursing staff on the RAI process.
• Participate in Medicare Part A in-house reviews as directed.
